GETTING STARTED
1. For medically-based care, obtain a prescription for occupational therapy services from your child’s pediatrician with a diagnosis code. The physician’s order should state, “evaluate and treat.” As a parent or guardian, you can request a physician’s referral to Youngsters Occupational Therapy, LLC. This is not required to access educationally-based services.
2. Schedule an appointment for the occupational therapist to come to your home or a community setting. Please keep in mind that services are only offered in Fort Myers, Cape Coral, Sanibel Island, Estero, Marco Island, Bonita Springs, and Naples areas within the state of Florida.

3. Fill out the Welcome Packet. This will be provided to you prior to the initial appointment.
THE INITIAL APPOINTMENT
1. Give the Welcome Packet to your child’s therapist. The occupational therapist reviews it with you and answers any questions. Additional information may be requested in order to better understand your expectations for therapy, as well as the needs of your child and family.
2. You and/or educational providers will complete one or more questionnaire(s) that help the therapist assess your child’s abilities.
3. Your child’s occupational therapist administers an assessment or observes your child’s performance during various activities.
AFTER THE INITIAL APPOINTMENT
1. Your child’s therapist reviews all information and scores assessments. One or more follow-up evaluation appointment(s) may be scheduled to perform additional testing or observation.
2. Within 30 days of your child’s last appointment, assessments are interpreted and an evaluation report is completed.
3. Your child’s occupational therapist schedules an appointment to go over the evaluation results. If therapy is recommended, this will be discussed in collaboration with you.
